Downloadable Forms

 

With the exception of the Adobe Acrobat forms, the following standard Civil Service forms can be downloaded to a disk and then completed on the user's PC. To download a form, click on the desired title below, then follow the prompts given by your browser. (On most browsers, you will need to choose the "Save to Disk" option, and then choose the drive and directory you wish to save to.)

To view the Adobe Acrobat forms, you must have the free Adobe Acrobat Reader.
Note:  The Adobe Acrobat Reader only allows you to view and/or print documents. The Reader does not allow you to save documents.  

These forms have been provided as a public service and have all been successfully processed on a State computer system. However, we cannot guarantee success for all users, due to the wide variety of browsers/platforms in use. It is still the responsibility of the user to use these forms appropriately and in a manner consistent within these limitations. No actual text of these forms may be changed, and doing so may render them null and void. All relevant deadlines still apply.

Certification Record Change Request Form

  • This form is used to notify Civil Service of changes to your name, address, parishes where you are available to work, and your preference for permanent employment offers only, or both permanent and temporary employment offers.  This form is also used to request activation of inactive grades, and extension of expired grades.

Employment Application (SF-10)

  • This is the standard Civil Service job application, used to apply for all classified State jobs. For more information, see How Do I Apply for a State Job?, which contains other methods for obtaining an application.
